If we can enable patients to better care for themselves, and increase the NHS's ability to enhance remote healthcare provision, we will significantly reduce demand on the hospital system.

Our story started in 2015, producing videos for clinicians to support their patients. Since then we have grown to become the UK's largest provider of health information videos and the people behind the national and personalised Health and Care Video Libraries.

Our extensive experience in transforming pathways of care and our passion to empower patients with the knowledge and tools needed to manage their own health, has led us to develop CONNECTPlus. Built on both patient and clinical need and designed flexibly for organisations to make it their own, our unique, multiple conditions app offers a practical and quick to implement solution to the healthcare capacity challenge.
